smtp: fix EOB handling
SMTP automatically appends a \n.\n to an upload if there is not already
one at the end of the input. The implementation had a bug where this did
not happen, depending on read size and buffering.

Change test 900 to reproduce the failure. The bug only happened for mail
body input of known length, where EOS was known on the last chunk read.
Change test 900 to use an input file and make it large enough.

lib: stop overriding system printf symbols
After this patch, the codebase no longer overrides system printf

`curl_m*printf()` or the system ones using their original names.

Also:
- drop unused `curl_printf.h` includes.
- checksrc: ban system printf functions, allow where necessary.

lib: replace getsock() logic with pollsets
`getsock()` calls operated on a global limit that could
not be configure beyond 16 sockets. This is no longer adequate
with the new happy eyeballing strategy.

Instead, do the following:
- make `struct easy_pollset` dynamic. Starting with
a minimal room for two sockets, the very common case,
allow it to grow on demand.
- replace all protocol handler getsock() calls with pollsets
and a CURLcode to return failures
- add CURLcode return for all connection filter `adjust_pollset()`
callbacks, since they too can now fail.
- use appropriately in multi.c and multi_ev.c
- fix unit2600 to trigger pollset growth

pingpong: on disconnect, check for unflushed pingpong state
When a pingpong based protocol tries to perform a connection disconnect,
it sends a sort of "logout" command to the server, unless the connection
is deemed dead.

But the disconnect might happen before pingpong data has been completely
sent, in which case sending the "logout" will not work. Check the
pingpong state and do not "logout" when data is pending.

This was detected as a condition in fuzzing that triggered a debug
assert in the pingpong sending.

curlx: move into to curlx/
Move curlx_ functions into its own subdir.

The idea is to use the curlx_ prefix proper on these functions, and use
these same function names both in tool, lib and test suite source code.
Stop the previous special #define setup for curlx_ names.

The printf defines are now done for the library alone. Tests no longer
use the printf defines. The tool code sets its own defines. The printf
functions are not curlx, they are publicly available.

The strcase defines are not curlx_ functions and should not be used by
tool or server code.

dynbuf, warnless, base64, strparse, timeval, timediff are now proper
curlx functions.

When libcurl is built statically, the functions from the library can be
used as-is. The key is then that the functions must work as-is, without
having to be recompiled for use in tool/tests. This avoids symbol
collisions - when libcurl is built statically, we use those functions
directly when building the tool/tests. When libcurl is shared, we
build/link them separately for the tool/tests.

conn: fix connection reuse when SSL is optional
In curl 8.12 I tried to improve the logic on how we handle connections
that "upgrade" to TLS later, e.g. with a STARTTLS. I found the existing
code hard to read in this regard. But of course, the "improvements" blew
up in my face.

We fixed issues with imap, opo3, smtp in 8.12.1, but ftp was no longer
reusing existing, upgraded control connections as before. This PR adds
checks in our pytest FTP tests that verify reuse is happening as
intended.

I rewrote the logic in url.c again, so that the new test checks now pass.

smtp: TLS upgrade fix
There were two places in the code that tried to connect the SSL filter,
e.g. do the TLS handshake, but only one changed stmp state to EHLO
afterwards.

Depending on timing, the wrong path was taken and the connection was
hanging, waiting for a server reply to a command not sent.

Do the upgrade to tls in one place and update connection filter and
smtps protocol handler at the same time. Always transition to EHLO on

http: expect 100 rework
Move all handling of HTTP's `Expect: 100-continue` feature into a client
reader. Add sending flag `KEEP_SEND_TIMED` that triggers transfer
sending on general events like a timer.

HTTP installs a `CURL_CR_PROTOCOL` reader when announcing `Expect:
100-continue`. That reader works as follows:

- on first invocation, records time, starts the `EXPIRE_100_TIMEOUT`
timer, disables `KEEP_SEND`, enables `KEEP_SEND_TIMER` and returns 0,
  eos=FALSE like a paused upload.

- on subsequent invocation it checks if the timer has expired. If so, it
enables `KEEP_SEND` and switches to passing through reads to the
  underlying readers.

Transfer handling's `readwrite()` will be invoked when a timer expires
(like `EXPIRE_100_TIMEOUT`) or when data from the server arrives. Seeing
`KEEP_SEND_TIMER`, it will try to upload more data, which triggers
reading from the client readers again. Which then may lead to a new
pausing or cause the upload to start.

Flags and timestamps connected to this have been moved from
`SingleRequest` into the reader's context.

lib: Curl_read/Curl_write clarifications
- replace `Curl_read()`, `Curl_write()` and `Curl_nwrite()` to
clarify when and at what level they operate
- send/recv of transfer related data is now done via
  `Curl_xfer_send()/Curl_xfer_recv()` which no longer has
socket/socketindex as parameter. It decides on the transfer
setup of `conn->sockfd` and `conn->writesockfd` on which
  connection filter chain to operate.
- send/recv on a specific connection filter chain is done via
`Curl_conn_send()/Curl_conn_recv()` which get the socket index
  as parameter.
- rename `Curl_setup_transfer()` to `Curl_xfer_setup()` for
  naming consistency
- clarify that the special CURLE_AGAIN hangling to return
`CURLE_OK` with length 0 only applies to `Curl_xfer_send()`
and CURLE_AGAIN is returned by all other send() variants.
- fix a bug in websocket `curl_ws_recv()` that mixed up data
when it arrived in more than a single chunk

The method for sending not just raw bytes, but bytes that are either
"headers" or "body". The send abstraction stack, to to bottom, now is:

* `Curl_req_send()`: has parameter to indicate amount of header bytes,
  buffers all data.
* `Curl_xfer_send()`: knows on which socket index to send, returns
  amount of bytes sent.
* `Curl_conn_send()`: called with socket index, returns amount of bytes
  sent.

In addition there is `Curl_req_flush()` for writing out all buffered
bytes.

`Curl_req_send()` is active for requests without body,
`Curl_buffer_send()` still being used for others. This is because the
special quirks need to be addressed in future parts:

* `expect-100` handling
* `Curl_fillreadbuffer()` needs to add directly to the new
  `data->req.sendbuf`
* special body handlings, like `chunked` encodings and line end
conversions will be moved into something like a Client Reader.

In functions of the pattern `CURLcode xxx_send(..., ssize_t *written)`,
replace the `ssize_t` with a `size_t`. It makes no sense to allow for negative
values as the returned `CURLcode` already specifies error conditions. This
allows easier handling of lengths without casting.

CURL_DISABLE_FORM_API flag and the HTTPREQ_POST_FORM value

pingpong: stop using the download buffer
The pingpong logic now uses its own dynbuf for receiving command
response data.

When the "final" response header for a commanad has been received, that
final line is left first in the recvbuf for the protocols to parse at
will. If there is additional data behind the final response line, the
'overflow' counter is indicate how many bytes.
