lib: make resolving HTTPS DNS records reliable:
- allow to specify when they are wanted on starting a resolve
- match dns cache entries accordingly. An entry which never
  tried to get HTTPS-RRs is no answer for a resolve that wants
  it.
- fix late arrivals of resolve answers to match the "async"
  records that started them - if it still exists.
- provide for multiple "async" resolves in a transfer at the
  same time. We may need to resolve an IP interface while the
  main connection resolve has not finished yet.
- allow lookup of HTTPS-RR information as soon as it is
  available, even if A/AAAA queries are still ongoing.

For this, the "async" infrastructure is changed:

- Defined bits for DNS queries `CURL_DNSQ_A`, `CURL_DNSQ_AAAA`
  and `CURL_DNSQ_HTTPS`. These replace `ip_version` which says
  nothing about HTTPS.
  Use them in dns cache entries for matching.
- enhance the `async->id` to be a unique `uint32_t` for
  resolves inside one multi. This is weak, as the id may
  wrap around. However it is combined with the `mid` of
  the easy handle, making collisions highly unlikely.
  `data->state.async` is only accessed in few places where
  the mid/async-id match is performed.
- vtls: for ECH supporting TLS backends (openssl, rustls, wolfssl),
  retrieve the HTTPS-RR information from the dns connection filter.
  Delay the connect if the HTTPS-RR is needed, but has not
  been resolved yet.

The implementation of all this is complete for the threaded
resolver. c-ares resolver and DoH do not take advantage of
all new async features yet. To be done in separate PRs.

Details:

c-ares: cleanup settings and initialisation. Any ares channel
is only being created on starting a resolve and propagating
operations in setopt.c to the channel are not helpful.

Changed threaded+ares pollset handling so that they do not
overwrite each others `ASYNC_NAME` timeouts.

Add trace name 'threads' for tracing thread queue and
pool used by threaded resolver.

dnscache: own source file, improvements
- Rename `Curl_resolv_unlink()` to `Curl_dns_entry_unlink()`.
- Change `Curl_dnscache_get()` to return CURLcode result. Returns
  now `CURLE_COULDNT_RESOLVE_HOST` for "negative" cache entries.
- Add `Curl_dnscache_add_negative()` to put a "negative" entry
  into the cache.

lib: move all UNITTEST prototypes to C files
- make extract-unit-protos handle multi-line prototypes - but they need
  to be above the implementation

- Prototypes for static functions we use in unit tests should not be in
  header files. We generate lib/unitprotos.h for this purpose

- Removed some function wrappers written for unit tests and make them
  use UNITTEST function directly.

- Renamed time2str() in the tool to timebuf() since we have the same
  name in lib/ and in unit tests they can both be used non-static in a
  build.

This reverts commit f95fadd116.

`readability-inconsistent-declaration-parameter-name`.

- do not enforce the above because of inconsistencies still present
  between public API prototypes and definitions. (Also betwen man page

build: assume stdint.h
curl requires `stdint.h` from C99, and no longer builds without it since
v8.18.0 (after dropping VS2008 support). Assume it's available, drop
feature checks.

Also:
- drop duplicate `stdint.h` includes.
- introduce internal `HAVE_UINTPTR_T`, enabled by default.
- OS400: disable `HAVE_UINTPTR_T`.
- build: keep cmake pre-fill and `cmp-config.pl` exception because cmake
  and autotools both detect `stdint.h` implicitly.

hostip: make more functions return CURLcode
- Curl_async_getaddrinfo() always returned NULL so it was pointless.
  Return proper curlcode instead to distinguish between errors. Same for
  Curl_doh().
- simplify the IP address handling
- make Curl_str2addr() function return CURLcode

doh: rename symbols to avoid collision with mingw-w64 headers
Collision happens when building with mingw-w64 v3 or v2 while targeting
Vista or newer. `iphlpapi.h` includes `windns.h` in this case, which
defines macros named `DNS_TYPE_*`, colliding with curl doh enums.

The issue was fixed in mingw-w64 v4:

multi: do transfer book keeping using mid
Change multi's book keeping of transfers to no longer use lists, but a
special table and bitsets for unsigned int values.

`multi-xfers` is the `uint_tbl` where `multi_add_handle()` inserts a new
transfer which assigns it a unique identifier `mid`. Use bitsets to keep
track of transfers that are in state "process" or "pending" or
"msgsent".

Use sparse bitsets to replace `conn->easyq` and event handlings tracking
of transfers per socket. Instead of pointers, keep the mids involved.

Provide base data structures and document them in docs/internal:
* `uint_tbl`: a table of transfers with `mid` as lookup key,
   handing out a mid for adds between 0 - capacity.
* `uint_bset`: a bitset keeping unsigned ints from 0 - capacity.
* `uint_spbset`: a sparse bitset for keeping a small number of
  unsigned int values
* `uint_hash`: for associating `mid`s with a pointer.

This makes the `mid` the recommended way to refer to transfers inside
the same multi without risk of running into a UAF.

Modifying table and bitsets is safe while iterating over them. Overall
memory requirements are lower as with the double linked list apprach.

async: DoH improvements
Adds a "meta_hash" to each easy handle for keeping special data during
operations. All meta data set needs to add its destructor callback, so
that meta data gets destroyed properly when the easy handle is cleaned
up or reset.

Add data->master_mid for "sub" transfers that belong to a "master" easy
handle. When a "sub" transfer is done, the corresponding "master" can
add a callback to be invoked. Used in DoH name resolution.

DoH: use easy meta hash to add internal structs for DoH name resolution.
One in each in each probe easy handle. When probes are done, response
data is copied from the probe to the initiating easy.

This allows DoH using transfers and their probes to be cleaned up in any
sequence correctly.

Fold DoH cleanup into the Curl_async_shutdown() and Curl_async_destroy()
functions.

asyn resolver code improvements
"asyn" is the internal name under which both c-ares and threaded
resolver operate. Make the naming more consistent. Implement the c-ares
resolver in `asyn-ares.*` and the threaded resolver in `asyn-thrdd.*`.
The common functions are in `asyn-base.c`.

When `CURLRES_ASYNCH` is defined, either of the two is used and
`data->state.async` exists. Members of that struct vary for the selected
implementation, but have the fields `hostname`, `port` and `ip_version`
always present. This are populated when the async resolving starts and
eliminate the need to pass them again when checking on the status and
processing the results of the resolving.

Add a `Curl_resolv_blocking()` to `hostip.h` that relieves FTP and SOCKS
from having to repeat the same code.

`Curl_resolv_check()` remains the function to check for status of
ongoing resolving. Now it also performs internally the check if the
needed DNS entry exists in the dnscache and if so, aborts the asnyc
operation. (libcurl right now does not check for duplicate resolve
attempts. an area for future improvements).

The number of functions in `asyn.h` has been reduced. There were subtle
difference in "cancel()" and "kill()" calls, both replaced by
`Curl_async_shutdown()` now. This changes behaviour for threaded
resolver insofar as the resolving thread is now always joined unless
`data->set.quick_exit` is set. Before this was only done on some code
paths. A future improvement would be a thread pool that keeps a limit
and also could handle joins more gracefully.

DoH, not previously tagged under "asny", has its struct `doh_probes` now
also in `data->state.async`, moved there from `data->req` because it
makes more sense. Further integration of DoH underneath the "asyn"
umbrella seems like a good idea.

  handling to allow HTTPS RR parsing to fail.
- keep target, ipv4hints, ipv6hints, port and echconfig also
  when resolving via cares. We need to know `target` and `port`
  when evaluating possible ALPN candidates to not go astray.
- add CURL_TRC_DNS for tracing DNS operations
- replace DoH specific tracing with DNS, use doh as alias
  for dns in curl_global_tracea()

urldata: introduce data->mid, a unique identifier inside a multi
`data->id` is unique in *most* situations, but not in all. If a libcurl
application uses more than one connection cache, they will overlap. This
is a rare situations, but libcurl apps do crazy things. However, for
informative things, like tracing, `data->id` is superior, since it
assigns new ids in curl's serial curl_easy_perform() use.

Introduce `data->mid` which is a unique identifer inside one multi
instance, assigned on multi_add_handle() and cleared on
multi_remove_handle().

Use the `mid` in DoH operations and also in h2/h3 stream hashes.

doh: fix cleanup
When removing an easy handle that had DoH sub-easy handles going, those
were not removed from the multi handle. Their memory was reclaimed on
curl_easy_cleanup() of the owning handle, but multi still had them in
their list.

Add `Curl_doh_close()` and `Curl_doh_cleanup()` as common point for
handling the DoH resource management. Use the `multi` present in the doh
handles (if so), for removal, as the `data->multi` might already have
been NULLed at this time.

build: untangle UNITTESTS and DEBUGBUILD macros
- fix `DEBUGBUILD` guards that should be `UNITTESTS`, in libcurl code
  used by unit tests.
- fix guards for libcurl functions used in unit tests only.
- sync `UNITTEST` attribute between declarations and definitions.
- drop `DEBUGBUILD` guard from test `unit2600`.
- fix guards for libcurl HSTS code used by both a unit test (`unit1660`)
  and `test0446`.
- update an existing AppVeyor CI job to test the issues fixed.

This fixes building tests with `CURLDEBUG` enabled but `DEBUGBUILD`
disabled. This can happen when building tests with CMake with
`ENABLE_DEBUG=ON` in Release config, or with `ENABLE_CURLDEBUG=ON`
and _without_ `ENABLE_DEBUG=ON`. Possibly also with autotools
when using `--enable-curldebug` without `--enable-debug`.

- Stick to a single unified way to use structs
 - Make checksrc complain on 'typedef struct {'
 - Allow them in tests, public headers and examples

 - Let MD4_CTX, MD5_CTX, and SHA256_CTX typedefs remain as they actually
   typedef different types/structs depending on build conditions.

dynbuf: introduce internal generic dynamic buffer functions
A common set of functions instead of many separate implementations for
creating buffers that can grow when appending data to them. Existing
functionality has been ported over.

In my early basic testing, the total number of allocations seem at
roughly the same amount as before, possibly a few less.

cleanup: remove the 'numsocks' argument used in many places
It was used (intended) to pass in the size of the 'socks' array that is
also passed to these functions, but was rarely actually checked/used and
the array is defined to a fixed size of MAX_SOCKSPEREASYHANDLE entries
that should be used instead.

06d8f16b87 headers: end all headers with guard comment
Most headerfiles end with a /* <headerguard> */ comment, but it was
missing from some. The comment isn't the most important part of our
code documentation but consistency has an intrinsic value in itself.
This adds header guard comments to the files that were lacking it.
